REGISTRATION FORM

As part of our safeguarding, we need to have a register of

the details of all the the children who attend Sunday Club.

Below are links to two versions of the form.

1. 'WORD' format. This can be downloaded and completed on your computer.

                              This can then be emailed to Janet Wales via this link

2. PDF format.     If you do not have 'Word' on your computer, you can download

                              a PDF version. This will then have to be completed by hand

                              and returned personally to Janet.

                              If you cannot open the PDF file, download a program here

                              that will allow you to open it.
